Minimum Qualifications for a lease agreement:

·       Doctor of Optometry (OD) from an accredited optometry school

·       Valid state optometry license

·       Ophthalmologist (MD or DO) from an accredited school

·       Valid state ophthalmology license

·       General Liability Certificate

·       Professional Liability Insurance Certificate
